Please Read Our EVENT POLICY Below Prior To Submitting Your Request Form
    Only fill this out for child's birthday event
    Class Waiver 
    (Required for ages under 18 years old)
    I understand that I (if under 18 years of age, my child) will be using sewing equipment and notions that can cause injury. I consent to my son/daughter being given medical treatment for any injury while in the care of this sewing class. KS SCHOOL OF DESIGN accepts no liability for injuries suffered during sewing class.
    Event Policy
    OUR POLICIES: Please note KS School of Design requires a waiver form to be signed for all participants prior to attending a class with KS School of Design.  If under eighteen years of age, must have parent or guardian approve and signed.

    CANCELLATION & REFUND POLICY - PARTIES & EVENTS:
    • Need to cancel no problem.  You can cancel anytime before 7 days prior to the start date of the event date, KSSOD will process a refund within 10 business days.
    • Need to reschedule no problem.  You can reschedule anytime before 7 days prior to the start date of the event date. 
    • Should KSSD cancel for any reason, we will notify you with other date options suitable for your event. Or a full refund will be made within 10 business days. 
Invoice with payment option will be emailed once we receive your registration application.
